Root Cause of Installation Failure
Microsoft has confirmed a critical installation failure for its May 2026 Patch Tuesday cumulative update for Windows 11, identified as KB5089549. Users are encountering error code 0x800f0922, along with occasional errors 0x80240069 and 0x80240031. The company added this issue to the update’s known issues log on May 15, three days after the patch’s release.
The root cause is insufficient free space in the EFI System Partition (ESP), a small reserved partition that stores essential boot files. The update introduces significant Secure Boot infrastructure changes, including a new SecureBoot folder on eligible devices and automation scripts for enterprise certificate management. These additions increase the data written to the ESP during installation, causing the failure on systems where the partition is nearly full.

KB5089549 is a mandatory cumulative security update for Windows 11 versions 25H2 and 24H2, released on May 12, 2026. It includes the latest security fixes, quality improvements from the April preview update, and critical Secure Boot certificate changes. Because Windows attempts to install this update automatically, affected devices experience a particularly disruptive failure.
The update also addresses a separate BitLocker issue from the April 2026 update, which could force systems into recovery mode after boot file updates. Microsoft is rolling out the new Secure Boot certificates in phases, adding devices only after they demonstrate sufficient successful update signals to maintain a controlled deployment.
